The Care Coordination Supervisor is primarily responsible for ensuring adherence to the high-fidelity wraparound model of care coordination, ensuring quality care for children and families/caregivers representing diverse socioeconomic, racial, ethnic, and cultural backgrounds. The supervisor has a thorough understanding of local community resources and are skilled at developing and maintaining working relationships with staff, clients, and community partners. This position will be responsible for serving Region I.
The pay range for this position is $2,609- 79,175.95 annually based on experience. LSW/LPC is required for this role.
Essential Functions:
- Supervise and oversee activities of care coordination team members through various methods, including but not limited to team meetings/supervision and regular communications.
- Provide training for direct care coordinators in the home and community setting.
- Provide routine training, support and assistance to staff through individual and group conferences, assistance in analyzing case problems, and in improving their skills.
- Promote an ongoing culture of team member recognition and create an atmosphere of teamwork.
- Complete and maintain a high-fidelity wraparound training credential.
- Ensure completion of skill and competency-based training to supervise delivery of ICC and MCC. Review, provide feedback, and approve all required documentation – ensure completion of all required care plans, initial assessments and CANS, outcomes reports, etc.
- Ensures coordination and adherence to the on-call rotation
- Participate in ongoing fidelity review and monitoring system focused on consistent application of system of care principles, adherence to OhioRISE ICC/MCC planning process and service components.
- Collaborate with all relevant systems and key participants within systems pertaining to children or youth and family/caregiver being served to ensure the highest quality of services are being provided with a focus on ensuring family voice and choice.
- Ability to assess needs in crisis situations and inform the response.
- Respects confidential information. Ensures operations meet quality and compliance standards.
- Ensures documentation meets Medicaid standards.
- Evaluates the performance of staff members and recommends appropriate action to the manager.
- Participate in the review and selection of new staff, including but not limited to completion of position requisitions, applicant reviews, and interviews.
- All other duties as assigned.
Minimum Requirements:
- Bachelor’s degree in social work or counseling required, master's degree preferred.
- LSW/LPC required.
- Three years of experience providing coordinated supportive services/care coordination services required.
- Previous supervisory experience preferred.
- Experience working in the behavioral health field is required, preferably experience working with multi-system youth, or individuals with autism and/or developmental disabilities.
- Working knowledge of local, state, and federal regulations for services for behavioral health, developmental disabilities, and autism.
- Knowledge of high-fidelity wraparound services preferred (training provided post-hire).
- Excellent communication skills, both oral and written, demonstrating professionalism with children, youth, families/caregivers, colleagues, and partners.
- Excellent organizational skills with the ability to stay focused
- Ability to prioritize multiple tasks
- Demonstrated clinical knowledge of children with co-occurring developmental disorders and complex behavioral health issues

POSITION SUMMARY:
The Wraparound Care Coordination Supervisor is responsible for oversight of services for youth who are assigned to the High Fidelity Wraparound Tier of service within Pathways to Success. Responsible the selection, training and supervision of Wraparound Care Coordinators. Provides clinical consultation to program staff regarding clients as needed concerning program services. Assures compliance with HFS Care Coordination and Support Organization requirements and all regulations in which the services and agency are provided under.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- In coordination with Manager of Care Coordination, interviews, selects, trains and supervises Wraparound Care Coordinators. Monitors employee performance and completes quarterly and annual evaluations. Works to resolve any performance issues or concerns.
- Provides oversight to client referrals, intake and admissions, assuring timely response to referral sources.
- Maintains positive relationships and open communication with community providers and others with whom the agency collaborates on behalf of clients.
- Oversees record keeping and reporting processes to insure that flow of written communication is current and effective.
- Provides leadership to the Utilization Review committee to ensure that the process is completed and that results are used to improve the quality of services.
- Participates in the all required committees including but not limited to monthly staff meeting and leadership meeting.
- Responsible for all program reviews and the success of the programs key performance indicators.
- Ensure needs of clients are met and services maintain Wraparound fidelity. Ensure staff have a variety of skills and resources to utilize when working with youth and families.
- Monitors direct service requirements of staff to ensure targets are being met and monitors caseload size to ensure accurate ratio.
- Adheres to and follows all other required duties necessary and assigned by Manager.
QUALITICATION REQUIREMENTS

CARE & TCI
Hoyleton has adopted the CARE (Children and Residential Experience), developed by Cornell University , to guide our culture and practice. CARE is grounded in six core principles: Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed.
Completion of CARE and TCI training is an internal requirement for all staff and must be completed upon hire. Ongoing application of these principles is expected and essential to successful job performance.

Working Conditions

EDUCATION AND/OR EXPERIENCE
- A Masters Degree in Social Social Work, Counseling, or Psychology from an accredited program, or in a related human service field.

- Supervisory experience in the human services field is required
- Knowledge of community programs, services and evidence-based practices and a variety of treatment modalities used to work with youth and their family to navigate child serving systems

Optimize the functioning of individuals increasing self-sufficiency and satisfaction in the living, learning, work and social environment of their choice through evaluation, monitor, linkage, and advocacy. Provide services to adults and children suffering from a serious and persistent mental illness who have complex needs and will require services to maintain or improve level of functioning, self-sufficiency and independence for at least one year. Maintain case load of no more than 40 consumers; wherein adults are considered one consumer and children are considered two consumers. Evaluate and discuss with supervisor consumer’s continued program eligibility as establish by 65 E 15 Guidelines. Maintain documentation as per 65 E 15; Medicaid and other state and federal regulatory guidelines. Develop and maintain resource database to facilitate linkage, brokerage and access to resources for consumers. Provide goal oriented and individualized supports through assessment, planning, linkage, advocacy, coordination and monitoring. Maintain communication with agencies and resources within the community to facilitate self-sufficiency and independence for consumers. Develop and updates Comprehensive Assessment and Comprehensive Service Plans reflecting individualized measurable goals in a timely manner (within 30 days of initial contact). Monitors and updates Comprehensive Service Plans when there is a significant life/status change in consumer’s life but at a minimum of every six months from initial plan. Complete a home visit during the development of the Comprehensive Assessment and prior to the completion of the CSP; and conduct home visits at least every other month for every consumer provided that it is approved by the consumer. Prepares and maintain documentation in compliance with contracting and regulatory agencies and not limited to verbal instructions from direct supervisory staff. Maintains consistent productivity, at least 80% of the established agency goal, per month. Submit/file required documentation in the expected timely manner. Maintain face-to-face contact at least once per month with consumers. Assume responsibility for the maintenance and auditing of all assigned consumer’ Case Management records. Participate in Performance Improvement Program and Peer Review as required. Procure contingency funds as provided by DCF procedures.

Possess knowledge of community resources, Community-Based Organizations and private providers of services to optimize the functions of individual eligible to receive management services. About the Role

The Director of Care Coordination is a senior leadership role responsible for ensuring that the interactions and experiences patients, loved ones, and referral partners have with Plum Creek Recovery Ranch are exemplary and consistent. This is accomplished by collaborating with facility-based employees, business development staff, strategic referral partners, and patient support system points of contact. The director will manage and supervise onsite care coordinators and have additional reporting responsibilities to the CEO and other members of the facility leadership team. The Director of Care Coordination also focuses on maintaining and improving strategic referral partnerships by optimizing communication and strengthening business relationships. The ideal candidate has experience in behavioral health and/or SUD management as part of the treatment team. Experience as a case manager, care coordinator, or business development representative is preferred.
Requirements
Essential Duties:
Leadership & Departmental Supervision
• Management of onsite case management/care coordination team. Ensuring case managers/care coordinators are regularly meeting with patients for discharge planning, transition plans, and treatment support. Ensuring case managers are meeting KPIs as determined by facility and outreach leadership.
· Train and oversee clinical team in support of outreach corporate initiatives regarding client aftercare coordination with our preferred providers. This includes but is not limited to documentation in Kipu, Salesforce, and other areas as deemed appropriate.
· Primary contact between the facility staff, business development, and strategic partners in assuring interdepartmental communication and patient care coordination is completed in a timely and appropriate manner.
· Develop, implement, and manage operational policies, procedures, and performance metrics to ensure efficiency and effectiveness.
· Ensures, when appropriate, that we are referring patients to vetted, strategic partners for continued care.
· Attends weekly and appropriate treatment team and clinical meetings for important patient updates and discharge planning discussions.
· Coordinate tours with business development representatives and external referral sources, ensuring visitors have an experience in alignment with facility values of excellence.
· Facilitate aftercare groups and individual meetings with patients regarding discharge planning and continuation of care.
· Collaborate with family therapists to provide resources for families of patients.
· Work closely with the admissions team to identify strategic opportunities to refer patients to partner facilities when not appropriate for care at Plum Creek Recovery Ranch.
· Collaborate with alumni coordinator to identify any opportunities to provide additional resources and referrals to our patients and treatment partners.
Staff Management & Development
• Lead, mentor, and evaluate care coordinators, fostering a culture of accountability, teamwork, and continuous improvement.
• Ensure appropriate staffing levels, training programs, and professional development opportunities to maintain service excellence.
Compliance & Quality Assurance
• Ensure compliance with federal, state, and local licensing regulations, accreditation standards, and industry best practices.
• Oversee and monitor patient satisfaction, treatment outcomes, and departmental performance within the standards and best practices of the behavioral healthcare industry.
Community & Stakeholder Relations
• Represent the facility in professional networks, industry events, and community partnerships.
• Strengthen relationships with referral sources, regulatory agencies, and other key stakeholders.
Knowledge Skills and Abilities
Education & Experience
- Bachelor’s degree in healthcare administration, business administration, or related field. Years of experience may be substituted for a degree, depending on the type of experience.
- Minimum of 5 years in the behavioral healthcare industry, with at least 2 years in a management or lead role within a behavioral health, substance use, or residential treatment setting.
- Experience working with patients and documentation in a behavioral health facility is required.
Skills & Competencies
- Exceptional leadership, communication, organizational, and decision-making skills.
- An understanding of healthcare regulations, licensing, and accreditation processes (e.g., Joint Commission, CARF).
- Knowledge of KIPU, Salesforce, or other similar EMR and CRM software.
- Excellent interpersonal and communication skills, with the ability to work effectively across all levels of the organization.
- Must have basic PC skills that include a combination of working in a Windows Operating System and Microsoft Outlook, Word and Excel and the ability to type a minimum of 30 words per minute.
