This Program Manager will oversee three Home Hemodialysis(HHD) /Peritoneal Dialysis (PD) programs in the local area.

The programs are located in; Northern Indianapolis, Lafayette and Lebanon

PURPOSE AND SCOPE:

Manages patient care in home therapies programs while maintaining cost-effective clinical operations. Provides direction and guidance to the interdisciplinary team providing care to the Home Therapies patients to ensure that the highest standard of care is provided. Collaborates with the Business Unit management team to grow the assigned Home Therapies programs.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Manages the operations of multiple Home Therapies programs with direct responsibility for results, including growth, patient retention, program costs, operational methods, scheduling, and staffing.   
• Directs activities related to the area’s strategic plans, goals and objectives ensuring alignment with those of the region, division, and company. Implements quality goals and action plans to achieve quality standards.
• Collaborates with operations leaders, market development, in-center leadership and patient education and quality teams to promote home therapies growth in assigned programs.
• Responsible for profit and loss management of the assigned program including optimal performance of program operations to achieve or exceed budgets and key performance indicators.
• Directs and manages all aspects of patient care for Home Therapies patients in assigned programs from admission through discharge of the patient. Provides direction, guidance, and support to staff members to ensure the safe and effective delivery of home therapy support care to patients with End-Stage Renal Disease (ESRD). 
• Oversees patient and caregiver readiness and ability to perform dialysis treatments at home and assesses environment for suitability in the ongoing delivery of home dialysis therapy, including training patients and caregivers on the safe and effective operation and preventative maintenance of equipment.
• Manages patient experience efforts by supporting company patient satisfaction initiatives, embodying the company values and patient experience drivers while encouraging others to do the same.
• Ensures competency of staff in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone, maintaining appropriate staff licensures and CPR certifications.
• Responsible for management of program supplies, equipment tracking, ensures that orders are replenished in a consistent and timely manner.
• Directs activities related to the Continuous Quality Assurance Improvement, patient retention and Clinical Algorithm Adoption, patient access management and patient training success.
• Directs data entry related activities such as patient admission, regulatory reporting, billing, physician orders, lab results, vital signs, patient status and any other treatment related data into systems.
• Ensures regulatory, compliance, and audit activities are accomplished on time.
• Orients and supports all new home therapies staff and provides ongoing education, ensuring compliance with all risk management initiatives, including in-center staff.
• Responsible for hiring, coaching, and counseling employees, including performance reviews, disciplinary action, and terminations.
• Provides leadership, coaching, and development plans for all direct reports to maintain an engaged and productive workforce, partnering with Human Resources on employee matters.
• Performs other related duties as assigned.

PHYSICAL DEMANDS AND WORKING CONDITIONS :

• The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.
• The position provides direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation.  Equipment used can vary in complexity. Equipment aids and/or coworkers may provide assistance. 
• The employee may occasionally be required to move, with assistance, machines and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. up as high as 5 feet.  There is a two-person assist program and "material assist" devices for the heavier items.
• This position requires frequent, prolonged periods of standing and/or walking and the employee must be able to bend over.

SUPERVISION:

EDUCATION :

• Graduate of an accredited School of Nursing (R.N.) Advanced education preferred. Current appropriate state licensure.

EXPERIENCE AND REQUIRED SKILLS :  

• Minimum of one year nursing experience plus a minimum of six months dialysis nursing experience.
• 3+ years’ supervisory or project/program management experience preferred.
• Demonstrated management skills necessary to provide leadership in the supervision of dialysis personnel and to ensure the delivery of maximum quality care to all patients.
• Demonstrated leadership competencies and skills for the position, including excellent communication, customer service, continuous quality improvement, relationship development, results orientation, team building, motivating employees. Performance management and decision making.
• Successfully complete CPR Certification with maintenance as required.

POSITION SUMMARY/RESPONSIBILITIES

The Burn Program Manager is responsible for the coordination of the ABA (American Burn Association) Verified Burn Center. This leadership role ensures the delivery of high-quality patient care, compliance with ABA standards, and promotes multidisciplinary collaboration. The position will collaborate with Medical Directors, program leaders, and multidisciplinary teams to coordinate, monitor, promote, and evaluate the program’s components, oversee clinical operations, and lead quality improvement initiatives. The Burn Manager must possess clinical expertise in the burn patient population across the continuum of care, as well as demonstrate experience with program leadership.

EDUCATION/EXPERIENCE

• Bachelor’s degree in Nursing is required; Master’s degree in Nursing, Healthcare Administration, or related field is preferred.
li>Minimum 3 years of healthcare leadership experience, in trauma or burn care settings.
• Minimum 3 years of burn care experience, in an acute care setting.  Critical Care experience preferred.
li>Demonstrated competency in ABA burn center accreditation standards and burn standards of care.

The Quality Improvement Program Manager will work closely with the Health Plan Quality Leadership to coordinate quality improvement and compliance activities and ensure that we are meeting the organizational, state, NCQA, CMS and other requirements as applicable.  This position will serve as a liaison between departments within the health plan and remainder of the organization to coordinate an interdisciplinary approach to improve member outcomes and provider performance.  Primarily responsible for the development and/or on-going management and administration of clinical quality programs, development/management of performance improvement projects by leveraging data, lead analytical efforts to monitor performance on key quality metrics (e.g., HEDIS, CAHPS, Stars), proactively identify opportunities for improvement, and support cross-functional teams with actionable insights.

This role may require travel to support community events, possibly on the weekends.  The individual must be comfortable with creating presentations and working with data to formally present information to physicians, administrators, other providers and community partners.

This is a work from home / telecommute position within Oahu. Travel will be required to the Honolulu office for meetings, offsite Quality Committee meetings, and State sponsored meetings up to 10 percent of the time.

Primary Responsibilities:

• Develop and implement data-driven strategies to support quality improvement initiatives
• Maintain and enhance reporting systems for HEDIS, Stars, CAHPS, and other regulatory and internal quality metrics
• Conduct data analysis to identify trends, gaps, and opportunities for improvement
• Support quality improvement projects with data analysis, performance tracking, and outcome measurement
• Manages all Quality Committees under the UnitedHealthcare Community Plan Hawaii umbrella to assure that QM Committee activities are fulfilled, on track and recorded
• Serves as a liaison with national Quality Committees to communicate back information to the local plan as needed
• Support with quality management documents such as the Quality Improvement Program Description, Quality
• Improvement Work Plan and the Quality Improvement Program Evaluation are completed, presented for approval and submitted timely
• Evaluations are written and updated as required to fulfill requirements of UHC national QMP, NCQA and State regulations 
• Manages existing QMP policy & procedures, writes new QMP policy & procedures and matches policy and procedure to regulatory requirement and accreditation crosswalk, and ongoing regulatory and accreditation readiness
• Provide project management oversight for all activities associated with HEDIS, CAHPS, PIPs, and other projects as assigned
• Conducts performance improvement projects and complete submission of necessary reports.
• Participate in quality related audits (i.e. EQRO, HEDIS, etc.) and assures that all QMP requirements under the audit are met
• Manages quality related reports as required by State, NCQA, etc.
• Prepare effectiveness studies on program activities to identify improved outcomes and status of meeting set objectives
• Other duties as assigned

Required Qualifications:

• 3+ years of experience in the health care field with a focus on healthcare data analysis and quality improvement

• Solid understanding of HEDIS, Stars, CAHPS and other healthcare quality metrics
• Demonstrated excellent communication and project management skills

Preferred Qualifications:

• 1+ years of experience with HEDIS, Stars, CAHPS and other healthcare quality metrics 
• 1+ years of experience working in health plan or other setting in quality management
• Knowledge of regulatory agencies including NCQA
