The Community Health Worker is responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW will build trusting relationships with clients and to connect those clients to care, eliminating barriers and advocating for systemic changes along the way. The CHW supports providers and the Case Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of clients and their families. The CHW will work to guide clients facing the effects of poverty, and/or experiencing homelessness, toward more permanent primary care services outside the hospital system. Establishing a secure, trusted connection with a primary care provider can help prevent persons facing poverty and/or homelessness from reliance on Emergency Departments to meet primary care needs. The CHW will be assigned to assist clients seeking assistance, to help them sign up for Medi-Cal, and provide linkages to medical care, and health education opportunities, with the
support of the CalAIM Services Manager.

Responsibilities:

Under the direction of the Program Manager, the Community Health Worker will serve as the primary liaison to women in high-need geographic locations within the community. This position is in-person and the primary work scope will require recruitment, engagement, and support of individuals from within the identified target communities to increase health literacy, retain healthcare coverage, and maintain health and community supportive service care throughout the life course. 

Responsibilities will include, but may not be limited to:

·         Conduct neighborhood “on the ground” outreach and networking to find and connect with high-need individuals, with particular emphasis on those not yet engaged in mainstream service systems. 

·         Use client-centered approaches to identify individual client and family needs, goals, strengths, and challenges. 

·         Complete client risk assessment to determine need for and level of service and support.

   Communicate and work with the Program Manager on high need/high risk cases; follow guidance for working with individuals with specific issues or need areas.

·         For clients that are not enrolled in other home visiting programs, offer and provide regular home visits that include client-centered provision of health information, modeling and demonstrating skills, and reinforcing positive health choices and behaviors. 

·         Refer and provide direct 1:1 assistance to help clients obtain and consistently utilize health insurance (including FPBP), primary care and/or prenatal care services, family planning services, and other needed community services such as WIC, substance abuse, domestic violence, mental health, etc. 

·         Provide and disseminate written and oral information about available family planning health services in the community to prevent unintended pregnancies and promote spacing of subsequent pregnancies. 

·         Provide individualized social support to encourage and reinforce health-promoting behaviors by clients, including personal and family health behaviors, consistent use of effective contraception to prevent unintended pregnancy or support birth spacing, utilization of needed health and supportive services, and communicating their questions and needs to service providers.

·         Link families to other family resources within the community such as libraries, museums, Family Resource Centers, Child Care Resource and Referral agencies, play groups, breastfeeding support groups, etc. 

·         In collaboration with the Program Manager and convene or arrange group educational sessions for expectant and new families. 

·         Complete and input data entry for client enrollment, progress and outcomes.  

·         Assist as requested with reports.

Areas of Expertise:

<       Knowledge of the organization and delivery of health care services, NYS public health insurance programs, Family Planning Benefit programs, and Health Care Exchange Information.

<       Knowledge of community support systems and how to access these services.

<       Strong oral and written communication skills.

<       Flexibility and ability to work as a team member.

<       Computer proficiency – Word. 

Minimum Qualifications:

<       HS education or above and a strong understanding of the needs of women in the identified target area.  

<       1-5 years related work experience; preferably at least 2 years experience working with high need women or in a home visiting capacity

Under the direct supervision of the Enhanced Care Management Director, the Enhanced Care Management Member Navigator is a person with direct knowledge of the local community who functions as a “guide” and offers assistance to community members in “navigating” the health care and accessing related social and financial services to improve health outcomes. ● Conducts outreach and enrollment activities as directed to engage members identified as being ECM eligible. Works with care team staff to identify potential members to outreach and enroll into the ECM.

● Performs, as needed, member scheduling, program eligibility screening, broken appointment follow‐up, etc.

● Utilizes language and cultural awareness skills to outreach to members and community and present T.H.E.’s ECM Program in an acceptable manner.

Since our founding in 1991, we have operated under the mission of providing personalized, high-quality health services to patients across the life cycle, regardless of ability to pay. Our service population has always been primarily composed of patients who face unequal access to care because of their membership in historically underinvested communities. We provide comprehensive medical, dental, vision and behavioral healthcare services across the life span in the Northern California Counties of Butte, Sacramento, Sutter, and Yuba.

The Community Health Worker (CHW) functions as a member of a multidisciplinary team that conducts patient support to help achieve good health practices. Primary responsibilities include facilitation of health improvement, health promotion, health program development and support, disease prevention activities, and coordination with other partners within the community. CHWs assist clients by helping to resolve personal issues and identify resources to meet client needs. The CHW helps clients by providing support and counseling to individuals dealing with issues such as requiring basic behavioral health and medical care, substance abuse treatment, rehabilitation and navigating human relationships. Most CHW duties will be conducted in the community, on the streets, and on site at the clinic. The CHW will also lead and organize community group activities and outreach events. Pay and Benefits
