GHA Autism Supports is seeking qualified Substitute Direct Healthcare Tech I to join our team. Substitutes work on an as-needed basis. The right candidate for this position will be flexible, creative, motivated and self-directed in providing support to individuals with autism. Hours for the position may vary, and may require morning, holiday, and/or weekend availability. Preference will be given to candidates who are available to work on any shift (1st, 2nd or 3rd) and who are available to work 12-hour shifts on Saturdays and Sundays. Substitutes are expected work at least one shift every month, and must attend trainings as scheduled to maintain required credentials.
BASIC FUNCTION: To integrate GHA’s mission, vision and values by providing professional, person-centered guidance and support to individuals who have autism. This position provides personal or medical care as needed, teaches appropriate behaviors and provides behavior management assistance. Key Responsibilities:- Develop relationships with residents and their families using a consistent caregiver approach
- Perform all clinical duties in accordance with state nurse practice act and nursing standards of practice
- Promote resident's independence by establishing care goals and teaching self-care skills
- Monitor resident clinical and care conditions and act on changes to ensure patient comfort and safety
- Work alongside care associates to perform routine nursing care as needed
- Maintain infection prevention practices, administer medications, and perform treatments per orders
- Minimum 6 months experience as an RN
- Experience caring for seniors or seniors with cognitive impairment preferred
- Basic computer skills, including Microsoft Office, internet, and web applications
- Active RN license in the state of New Jersey and CPR certification required

Identify and address the strengths, which will aid the individual or family in the recovery and resiliency process, as well as the barriers that could impede the development of skills needed for independent functioning in the community. Support the individual and the family in crisis situations; provide personalized interventions to develop, facilitate, or enhance an individuals ability to make informed and independent decisions. The patient care assignment may include children who are at risk of/or experiencing serious emotional, neurobiological, and/or behavioral disorders; adults with severe mental illness (SMI), individuals with chronic substance abuse, and/or individuals with co-occurring disorders. Detailed responsibilities:
* PATIENT CENTERED MED - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable
* NAVIGATION/ PATIENT ADVOCACY - Work to eliminate barriers to access to culturally competent medical services for all patients. Assist patients to access community resources, and financial assistance programs and services that are needed
* COLLABORATION - Collaborate with medical providers, multidisciplinary team members and community resource providers to assist patient to meet above described treatment goals

* CRISIS INTERVENTION - Assess, support and intervene in crisis situations including the facilitation of the development and use of individual crisis management plans that recognize the early signs of crisis/relapse and use natural supports. Identify & encourage use of alternatives to hospital emergency departments and inpatient hospital services
* TRACKING - Coordinate and monitor use of services, including comprehensive tracking of client activities in relation to care plan such as attendance to all scheduled appointments, reviewing documentation of other in-house providers, and maintaining contact with external providers. Conduct post discharge follow-up contact as appropriate
* DOCUMENTATION - Document all client encounters and those made on behalf of clients; complete and submit billing documentation as appropriate; maintain current and comprehensive client files. Files may contain documents held for safe keeping on behalf of a client
