Position Summary
The BHC is responsible primarily for providing counseling to patients including walk-in clients. Experience & Training
- Masters’ Degree in counseling, psychology, or social work.

- Two - four years of experience in the field or a related area, clinical psychology preferred.
- Clinical psychosocial diagnosis experience involving direct patient care along with consultation, evaluation, and research preferred.
- Experience working with underserved, diverse and transient populations.
- Experience working with patients who have co-occurring mental health, substance abuse, and social & physical health challenges.

Essential Job Duties & Responsibilities
- Performs a variety of tasks in collaboration with a multidisciplinary team of physicians, psychologists, social workers, and other professional staff.
- Works evening and/or weekend shifts as required.
- Stays current professionally with all therapeutic interventions and assessment techniques to maintain clinical skills.
- Work with management on the integration of physical and mental health services to achieve departmental goals.
- Develops and maintains thorough knowledge of referral resources throughout the service area.
- Maintains up to date knowledge of billing codes and other third-party reimbursement sources.

Duties & Responsibilities :
- The Mental Health Consultant will develop, implement, and monitor the Mental Health Program to include staff development, mental health counseling of students, TEAP program counseling, according to the Job Corps federal regulations and the Policy and Requirements Handbook.
- The Mental Health Consultant provides on-call mental health services for emergencies, 24-hours a day, 7 days a week. MHC may provide services by phone or in person. May choose arrangements for services on behalf of the center.
- Responsible for the development, implementation and monitoring of the center’s Mental Health Program, including supervision of all mental health interns and employees.
- The Mental health Consultant advises the Center Director and Health Services staff of all mental health matters.
- Conducts staff development and training in the use of mental health principles and techniques.
- Provides consultation with Health Services staff and all other appropriate staff concerning student mental health issues.
- Develops constructive programs for meeting students’ mental health issues and works with center disciplinary, maximum benefits, intergroup relations and the AODA prevention programs.

- Issues and maintains Standing Orders on all mental health issues as required by the National Job Corps program.

Qualifications:

11. Provide educational materials, handout, and use teach back strategies tailored to patient’s specific issue.
12. Provide clear and concise feedback to Primary Care Provider, describe action plan and establish plan for follow up.
13. Promotes effective working relations and work effectively as part of a team to facilitate the Clinic’s ability to meet its goals and objectives.
14. Attains all agreed to goals and objectives within specified time frames, as part of the Clinic’s overall mission.
15. Responsibly follows the Clinic Exposure Control Plans/Bloodborne and Airborne Pathogens.

SUMMARY POSITION STATEMENT
The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages, and manages patients with medical and behavioral health problems within the primary care setting. In addition, the Clinician will provide skill training through psychoeducation and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations. Will manage own case load and provide psychotherapy to identified clients.
ESSENTIAL FUNCTIONS/ROLES & RESPONSIBILITIES OF THE POSITION
- Formulate behavioral health interventions appropriate to a primary care setting and assist with implementation and monitoring of treatment plans.
- Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
- Work as a team with providers to coordinate care for patients.
- Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
- Work with primary care teams to triage patients and provide referrals to specialty care behavioral health as appropriate.
- Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
- Assist in preventing relapses or morbidity in conditions that tend to recur over time.
- Teach patients, families, and staff care, prevention, and treatment enhancement techniques.
- Provide targeted assessments and evaluations, including diagnostic impressions and the utilization of screening tools.
- Provide timely and succinct feedback regarding consultation findings and recommendations.
- Provide Behavioral Health consultation services as needed when BHC is not available. As a Health Coach, provide immediate high-quality behavioral health services to new and existing patients in the medical clinic through coordination with medical providers using modalities such as motivational interviewing following an “always available, always interruptible” workflow.
- Provide training and guidance to primary care and specialty providers about the role of Health Coach.
- When appropriate, coordinate and provide additional behavioral healthcare to patients after initial hand-off from primary care provider.
POSITION REQUIREMENTS
Education: PhD or PsyD in clinical or counseling Psychology; Master of Social Work from an accredited graduate school of social work; master’s or doctoral degree in psychology or related field from a regionally accredited university.
License: Maintain a current, active license with the State of Alaska as, Licensed Clinical Social Worker (preferred), Licensed Professional Counselor, Licensed Marriage & Family Therapist or Licensed Clinical Psychologist.
Experience: Must possess specialized experience which involves using psychological assessments, formulating diagnosis or treatment recommendations, and providing counseling services to patients.

Roles and Responsibilities:
We are looking for a New York State Licensed Clinical Social Worker (LCSW)/Behavioral Health Consultant OR Licensed Psychologist/Behavioral Health Consultant to work in close concert with patients' medical providers and the integrated team to offer rapid access to brief mental and behavioral health assessment and intervention to help meet defined patient care goals:
- Training in one of the following fields (New York State licensure required):
- Clinical or Medical Social Work

- Pediatric experience/interest is preferred, although not required.
- Flexibility to adapt to fast-paced medical environments as a member of a dynamic care team.
- Demonstrated ability to collaborate and communicate effectively in a team setting.

- Experience with assessment and treatment planning for common mental health and/or substance use disorders, as well as working knowledge of differential diagnosis for these conditions.

- Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of primary care.
- Ability to effectively engage patients in a therapeutic relationship, when appropriate.
- Ability to work with patients by telephone / telehealth as well as in person.
- Familiarity with using electronic medical record systems.
- Training and / or experience in integrated behavioral health care (especially PCBH and CoCM) is a plus, but training in these care models will be provided for candidates without this specific background.

Overview
The Behavioral Health Consultant (BHC) is a vital member of our multidisciplinary team, delivering compassionate, trauma-informed care to individuals with complex behavioral health needs. This role focuses primarily on providing behavioral health consults within the clinic setting through the Primary Care Behavioral Health (PCBH) model, supporting patients engaged in medical care, and offering targeted interventions for substance use, particularly for participants in our Medication-Assisted Treatment (MAT) program. In addition to clinic-based services, the BHC facilitates individual and group therapy sessions at select Permanent Supportive Housing (PSH) sites. This position is ideal for a mission-driven professional passionate about integrated, person-centered care for underserved populations.
Duties and Responsibilities
Clinical Services
- Provide same-day behavioral health consults and brief therapy interventions during medical visits, utilizing evidence-based modalities (e.g., CBT, ACT, MI, solution-focused therapy).
- Conduct diagnostic assessments and develop individualized, strengths-based treatment plans.
- Deliver trauma-informed, culturally responsive care focused on reducing stigma and improving access to behavioral health services.
- Offer individual and group therapy at both Permanent Supportive Housing (PSH) sites and clinic settings, adapting care to co-occurring disorders and complex social needs.
- Implement crisis intervention strategies and safety planning for patients at risk of harm to self or others.
- Facilitate psychoeducational classes and skill-building group visits to promote emotional regulation, resilience, and wellness.
- Provide individual and group substance use counseling to participants in the Medication-Assisted Treatment (MAT) program.
- Support recovery-focused treatment through relapse prevention planning, recovery goal setting, and integrated care coordination with medical and behavioral health teams.
- Participate in interdisciplinary MAT case reviews and ensure continuity of care across service domains.
Care Coordination and Documentation
- Work closely with primary care and specialty medical providers to address psychosocial factors impacting health outcomes.
- Contribute to holistic treatment planning by integrating behavioral health interventions with primary care workflows.
- Apply brief, actionable interventions to enhance readiness for change and treatment adherence.
- Collaborate with case managers, housing teams, and community service providers to promote housing stability, health equity, and social support access.
- Participate in interdisciplinary case conferences and collaborative care meetings to ensure whole-person, team-based care.
- Serve as liaison between behavioral health, housing, and healthcare systems to reduce barriers to services.
- Ensure accurate, timely, and comprehensive documentation in the electronic health record (EHR), including assessments, treatment plans, and progress notes.
- Maintain compliance with clinical protocols, ethical standards, and regulatory guidelines (HIPAA, Medicaid, grant-specific requirements).
- Track patient outcomes and engagement metrics to inform program improvement and evaluate clinical effectiveness.

Minimum Qualifications
- Masters degree in Social Work, Counseling, Psychology, or related field
- Licensed or license-eligible (LCSW, LPC, LMFT, etc.) in the state of Texas.
- 2 years experience providing therapy and/or substance use counseling
- Strong clinical assessment, communication, and crisis intervention skills
- Ability to work effectively in community-based and clinic settings
Preferred Qualifications
- Bilingual in English/Spanish
- Experience working with PSH, MAT, or integrated care populations
- Familiarity with trauma-informed care and harm reduction principles
- Manages time effectively, handles multiple assignments simultaneously, and prioritizes projects

Job Purpose:? As a Behavioral Health Consultant (BHC) at Aspire Health Partners, your primary responsibility is to provide clinical expertise, as subject matter experts, and assist with the identification of parents with behavioral health conditions in the child welfare system and how it will impact child safety.

- Participate in ongoing data reporting, quality improvement efforts, and attend relevant meetings to contribute to the development of effective interventions for family safety and well-being
- Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures
Position qualifications:
- Minimum of 1yr experience working Substance Misuse and Mental Health population and making clinical recommendations.
- Working knowledge of community mental health and substance use providers and best practices for engagement and treatment.
- Knowledge of the child welfare and behavioral health systems preferred.
- Knowledge and understanding of the impact of mental health conditions and substance use disorders on parenting ability and child safety.
- Strong documentation, communication, and critical thinking skills required.
- Level II Background clearance
- Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1
- Master Degree in a human services field.
